Initial Comprehensive, Complex New Patient Appointment:( Complex Medical conditions such as Crohn's, U.C., MS, Chronic Lyme Disease, Chronic Fatigue Syndrome and Neurodegenerative Conditions)

75-90 Minutes 1:1 virtual

Pre-appointment review of all medical records, tests, and questionnaires.

Assessment, testing, diagnosis and treatment plan.

COST: $750.00

We do not bill insurance or Medicare. ( We will create a superbill that you can submit to your insurance company)

We accept HSA

for billing and supplements.

Established Patient Appointment:

45 minutes 1:1 virtually

Review of all test results, update treatment plan

COST : $275/45 minutes

Each additional 15 minutes will add on $75.00

Cash based, HSA accepted.

Membership:

1. 3 -Month Membership, consecutive months, and will only be canceled or terminated by the patient, or the request of the patient, after the minimum time has passed.

   COST: $200/ 45 minutes/ month for a minimum of 3 months. Recommended for patients who are established patients and have advanced testing completed and/or have multiple medical conditions. In depth review of all advanced laboratory tests, review of detailed recommendations for lifestyle modifications ( dietary, sleep, stress and exercise habits). Also includes adjustments of supplements, pharmaceutical medications and renewal of medications.

2. Yearly Membership, recommended for new patients and includes a yearly annual 60 minute visit, 8 follow up visits, unlimited patient portal emails, same day appointments, 20 % discount on all supplements.

   COST: $2,400.00/ annually

New Patient: One functional medicine test review, interpretation and treatment plan. ( We can pre-order test, cost not included)
